Headset knackered?

I was lucky? enough to reclaim by bike this week from Cash Generator after it had been stolen 3 months ago. It looks like it was fully stripped before the plonkers decided to put half the components back on (badly). The headset seems to be the worst of the bunch so i’m starting with that.
The handlebars were hard to turn so i had the stem off and discovered massive amount of sideways play in the headset, i’ve not really taken a headset apart since before aheadsets were a thing so not entirely sure what im looking at. Could i get some advice on weather its fixable or if i need to plonk a whole new one in? Not sure if it just needs new bearings (where would i get these) or if there are parts missing. Im about to have a good search for any diagrams specific to this model once this is posted.
The bike is a 2010 Cannondale Trail sl5 and the specs list the headset as simply Cane Creek 1.5″.

Looking at the state of that top tapered race I would say it is completely Knackered. Just fit a new one, it is really easy to do. a lackey’s tool kit will suffice ( hammer and a screwdriver) 😆 😆

Awesome thanks. Could i ask for a little help on identifying a replacement?
A headset is literally the only thing i have never changed or upgraded myself. And there seems to be a mind boggling array of specs to match.
Things i have picked up so far is that it may be a semi integrated?

If you google the code on your headset as seen in your picture. (OPFZSRBR) it will bring all the answers you need. 8) 8) 8)
